London Borough of Bexley is seeking providers to deliver Care at Home and Reablement Services across various locations, supporting people across the London Borough of Bexley to remain independent in their own homes, including through Care at Home and Reablement services.
Care at Home focuses on proactive, personalised support in the individual’s home, addressing assessed needs, social-care-related quality of life and wider wellbeing. Reablement focuses on enabling timely discharge from hospital, supporting people to remain at home and building confidence and ability to increase or maintain independence.
Suppliers can bid across multiple geographical areas and service types, subject to award limitations.
